Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with pairing kava and rifampin: both can be tough on your liver. Rifampin is well known for raising liver enzymes and, in some people, causing liver inflammation. Kava has been tied to over 100 reports of liver injury, mostly with heavy or long-term use. Stacking two liver stressors together may add up and raise your risk.

I want to be honest that this is a theoretical interaction based mostly on case reports, not strong studies. Still, since you'd already be watched for liver issues on rifampin, adding kava isn't worth it. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your doctor or pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Rifampin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with rifampin: the concern on file is whether kava might slow down CYP3A4, an enzyme that helps process rifampin. The honest answer is that this is shaky. Older lab (test tube) studies hinted kava could block this enzyme, but a better human study showed kava had no real effect on CYP3A4 at all. So this interaction is rated minor and unlikely.

The bigger thing I'd actually flag is your liver. Rifampin can stress the liver, and kava has been tied to rare but serious liver injury. Stacking two liver concerns isn't ideal. Please check with your pharmacist or doctor before using them together, and don't change your rifampin on your own.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
