CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Budesonide
Budesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: your body relies on a liver enzyme called CYP3A4 to break down budesonide. In a test tube, kava looked like it might slow that enzyme, which in theory could let more budesonide build up and raise the odds of steroid-type side effects (think higher blood sugar, fluid retention, or mood changes). But when this was actually studied in people taking kava twice daily for about a month, it had no real effect on CYP3A4. So this concern is mostly theoretical and unlikely to cause a problem.
The bigger thing I'd flag is the liver. Both kava and budesonide can stress the liver in rare cases, so combining them deserves a quick chat with your pharmacist or doctor before you start.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

What Is Budesonide Used For?
Brand names: Entocort EC, Pulmicort, Uceris, Tarpeyo, Eohilia
Budesonide calms inflammation. Which condition it treats depends on the product and how it is delivered.
- Asthma: inhaled budesonide (Pulmicort Flexhaler for ages 6 and older; nebulized suspensions such as Pulmicort Respules for children 12 months to 8 years) is used for long-term control.
- Crohn’s disease: budesonide delayed-release capsules treat mild to moderate active disease of the ileum and/or ascending colon (ages 8 and older). They also help maintain remission for up to 3 months in adults.
- Ulcerative colitis: Uceris extended-release tablets and Uceris/budesonide rectal foam help bring on remission of mild to moderate disease.
- Eosinophilic esophagitis (EoE): Eohilia oral suspension is used for 12 weeks in people 11 and older.
- IgA nephropathy (IgAN): Tarpeyo is used in adults at risk of progression, to reduce loss of kidney function.
The inhaled products are not for sudden asthma attacks.
