HEPATOTOXIC DRUGS

Doxycycline falls under HEPATOTOXIC DRUGS — the group Kava interacts with.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: this isn't about kava changing how your body handles doxycycline. Instead, it's about both of them being a little hard on the liver. Doxycycline can occasionally stress the liver on its own, and kava has been tied to rare cases of liver injury (usually with heavy, long-term use). Stacking two liver stressors together may, in theory, raise the odds of a problem.
I'll be honest that this concern is based on weak, anecdotal evidence, so it's more of a caution than an alarm. Still, watch for warning signs like yellowing of the eyes or skin, dark urine, belly pain on your right side, or unusual tiredness. If those show up, or before combining these, please check with your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES
2 of 2Doxycycline also falls under P-GLYCOPROTEIN SUBSTRATES.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for doxycycline and kava. Doxycycline is partly moved around your body by a pump called P-glycoprotein, and kava might (in theory) slow that pump down. If that happened, slightly more doxycycline could hang around, which could mean a bit more of its usual side effects like stomach upset, nausea, or sun sensitivity.
But honestly, this concern is weak. It comes from lab dish studies, not from people, and a human study using kava saw no real effect on a similar drug (digoxin). So for most folks this is unlikely to cause trouble. The bigger thing I'd flag is kava's rare risk of liver irritation, and doxycycline can occasionally stress the liver too. Run the combo by your pharmacist or doctor first.

Doxycycline is an antibiotic. It treats infections caused by bacteria, and it does not work on viruses.
Several products carry a broad list of uses, including rickettsial infections, sexually transmitted infections, respiratory tract infections, eye infections, and anthrax after exposure. Some product labels also list malaria prevention, and use as an add-on treatment for acute intestinal amebiasis and severe acne. Doxycycline for Injection (Doxy 100, intravenous) is approved for infections caused by a listed set of microorganisms. Doxycycline capsules in the Oracea-type low-dose form are described in the evidence for inflammatory lesions of rosacea.
