Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the thing with kava and nifedipine: the concern isn't really about how the two work in your body chemically. Instead, it's about the liver. Kava has been tied to rare cases of liver injury, and nifedipine is also processed through the liver. The worry is that stacking two things that can stress the liver might add up, especially with heavy or long-term kava use.

I'll be honest, this is a theoretical caution based on scattered case reports, not strong proof. Still, watch for signs your liver isn't happy, like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Before combining these, please check with your pharmacist or doctor first.

Nifedipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the gist with kava and nifedipine. Your body uses an enzyme called CYP3A4 to break nifedipine down. The worry on paper is that kava might slow that enzyme, which could let nifedipine build up and cause more of its usual effects, like dizziness, flushing, headache, swollen ankles, or blood pressure dropping too low.

The good news: the better-quality human research doesn't back this up. In a real study, people taking kava daily for about a month showed no meaningful change in CYP3A4 activity. So this is mostly a lab-dish concern, not something that reliably happens in people. A bigger issue with kava is its rare risk of liver harm. Before combining them, just check with your pharmacist or doctor.

What Is Nifedipine Used For?

Brand names: Procardia XL, Adalat PA, Adalat, Nifedin, Nifed PA, Nifed, Procardia, Adalat XL, Adalat CC, Adalat LA, Adalat Retard, Adipine MR, Adipine XL, Cardilate MR, Coracten SR, Coracten XL, Fortipine LA 40, Hypolar Retard 20, Nifedipress MR, Nifopress Retard, Tensipine MR

Nifedipine treats vasospastic angina (chest pain caused by a spasm in a heart artery) and chronic stable angina (chest pain that comes on with effort). It can be used for chronic stable angina when other treatments, such as beta blockers or nitrates, haven't worked well enough or can't be tolerated.

Nifedipine extended-release tablets, including Procardia XL, are also used to treat high blood pressure. The nifedipine capsule label covers the angina uses only.
