CNS DEPRESSANTS

Zonisamide falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind with zonisamide and kava: both of them have a calming, slowing effect on your brain. Zonisamide already tends to cause drowsiness, dizziness, trouble concentrating, and sometimes a bit of unsteadiness. Kava works in a similar direction (it's thought to act on GABA, the brain's quieting signal), so stacking the two together can pile those effects on top of each other.
In real life, that could mean feeling extra sleepy, foggy, or wobbly on your feet, which raises the risk of falls or slowed reactions. There's also a separate concern worth flagging: kava has been tied to rare liver problems, so it's not something to add on your own. Please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Zonisamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version on combining kava with zonisamide: the worry on paper is that kava might block CYP3A4, one of the liver enzymes that helps clear zonisamide. If that happened, more drug could linger in your system and you might feel extra drowsy, dizzy, or foggy. But the better-quality human research tells a calmer story. In a study of healthy volunteers taking 1000 mg of kava twice daily for a month, kava had no real effect on CYP3A4. So this is mostly a theoretical, lab-dish concern.
My bigger flag is actually the liver. Both kava and zonisamide can stress it, so doubling up makes me uneasy. Please check with your pharmacist or doctor before pairing them.

Zonisamide is used along with other seizure medicines (adjunctive therapy) to treat partial seizures. These are seizures that start in one area of the brain.
Zonegran and zonisamide capsules are for adults with epilepsy. Zonisade, the oral suspension, is for adults and teens 16 years and older.
