Why this applies to Finasteride

Finasteride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body uses a liver enzyme called CYP3A4 to help break down finasteride. On paper, some lab tests hinted that kava might slow that enzyme down, which in theory could let a bit more finasteride hang around. If that happened, you might notice slightly more of the usual finasteride effects, like changes in sex drive or mild breast tenderness. Finasteride is not a prodrug, so it works as-is.

The good news: a real-world study in healthy people taking kava daily for nearly a month found no meaningful effect on CYP3A4. This interaction is rated minor and unlikely. The bigger concern with kava is actually rare liver injury, so please check in with your pharmacist or doctor before combining them.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

About Finasteride

What Is Finasteride Used For?

Brand names: Propecia, Proscar

Finasteride tablets (including the brand Proscar) are used in men with an enlarged prostate (benign prostatic hyperplasia, or BPH) to improve urinary symptoms, lower the risk of acute urinary retention (a sudden inability to urinate), and reduce the need for prostate surgery. Finasteride tablets may also be used alongside the alpha-blocker doxazosin to slow the progression of BPH symptoms.

Propecia (and finasteride 1 mg coated tablets) is indicated for the treatment of male pattern hair loss (androgenetic alopecia) in men only. Propecia is not approved for use in women, and its effectiveness for hair loss at the temples has not been established. Finasteride is not approved for the prevention of prostate cancer.
