Triamcinolone falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the short version on combining kava with triamcinolone. Your body uses a kind of cellular pump called P-glycoprotein to move certain drugs around and out, and triamcinolone is one of the medicines that can ride on this pump. The thinking is that kava might slow that pump down, which in theory could let a bit more triamcinolone hang around in your system and slightly raise the chance of steroid side effects like fluid retention, mood changes, raised blood sugar, or stomach upset.

I want to be honest about how shaky this is. The concern comes only from lab dish studies, and a real human study found kava didn't change levels of a similar pump drug. So this is mostly theoretical. Still, one extra thing worth raising: both kava and steroids can stress the liver, so check with your pharmacist before pairing them.

Triamcinolone is available in many forms — injections, creams, ointments, nasal sprays, dental pastes, and eye injections — each approved for specific uses. Injectable forms given into muscle are used when oral steroids aren't an option, covering severe allergies (like asthma flares, severe skin reactions, drug reactions, and serum sickness), serious skin diseases, certain endocrine (hormonal) disorders, and more.

Injections given directly into a joint (intra-articular) or soft tissue help with conditions like rheumatoid arthritis, gout flares, bursitis, and tendon inflammation. Injections into skin lesions (intralesional) treat conditions such as keloids, alopecia areata, and psoriatic plaques. Triesence (an ophthalmic injection) is specifically indicated for eye conditions like uveitis and temporal arteritis, and for visualization during eye surgery. Topical creams, ointments, lotions, and sprays relieve itching and inflammation from corticosteroid-responsive skin conditions. Dental paste forms (Oralone, Kourzeq) treat painful mouth sores from injury. Nasal sprays temporarily relieve hay fever and upper respiratory allergy symptoms such as runny nose, sneezing, and congestion.
