Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the thing to know about pairing kava with Rozerem (ramelteon). Both of these can calm down your nervous system. Ramelteon works gently on your sleep signals to help you drift off, while kava adds its own relaxing, sedative pull. Put them together and those effects can stack up.

What that looks like in real life is feeling more drowsy or groggy than you'd expect, slower reactions, and feeling unsteady or foggy if you get up during the night. There's a separate concern too: kava has been tied to rare liver problems, so it's worth being careful overall. I wouldn't combine these on your own. Please check with your pharmacist or doctor first so they can guide you safely.

Theoretically, kava might increase levels of CYP2C9 substrates.
In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.

Here's the situation with kava and Rozerem. Your body uses a liver enzyme called CYP2C9 to help break down ramelteon. In test tube studies, kava appears to slow that enzyme down. If that same effect happens in people, ramelteon could clear more slowly and build up a bit, which might leave you feeling more drowsy, groggy, or dizzy, especially the next morning.

I want to be honest about the strength of this: it's only been seen in lab research, not in actual patients, so it's theoretical for now. There's also a bigger picture worry, since both kava and a sleep aid can add to sedation, and kava itself has been tied to rare liver problems. Please check with your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Your body uses an enzyme called CYP1A2 to break down Rozerem (ramelteon). The thinking here is that kava might slow that enzyme down, which could let ramelteon build up a little and make you feel extra drowsy, groggy, or foggy the next morning. The good news is that the evidence for this is shaky. A test-tube study and one case report hinted kava might block CYP1A2, but a better human study found kava had no real effect on it at all.

So on paper this is a minor and unlikely concern. I'd still mention it to your pharmacist, mostly because both kava and a sleep aid can add to sleepiness, and kava itself can rarely stress the liver. A quick chat before combining them is the safe move.

Here's the short version on combining kava with ramelteon (Rozerem). Ramelteon is processed in your body by a liver enzyme called CYP3A4. In a test tube, kava looked like it might slow that enzyme down, which in theory could let ramelteon build up and make you extra drowsy or groggy the next morning. But when researchers actually tested this in real people taking kava for a month, kava had no meaningful effect on CYP3A4. So this concern is mostly theoretical and unlikely to cause real trouble.

My bigger caution is a different one: both kava and ramelteon are sedating, and kava has been tied to rare liver injury. Please check with your pharmacist or doctor before pairing them.

Ramelteon tablets — sold as the brand Rozerem and as generic ramelteon — are used to treat insomnia when the main problem is trouble falling asleep at the start of the night. It is not indicated for difficulty staying asleep.

Clinical studies supporting its effectiveness ran up to six months in both adults and elderly patients.
