Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

The piece of CoActifed I want to focus on here is the codeine. Codeine is an opioid that already tends to make people sleepy and slow their breathing a bit. Kava has its own calming, sedative effect on the brain, likely through GABA pathways. Stack the two together and those effects can add up, leaving you groggier, more lightheaded, and slower to react than you'd expect. In serious cases that combined drowsiness and slowed breathing is what worries me most.

This is rated a major interaction with solid evidence, and guideline groups specifically warn against mixing kava with brain-calming drugs. Driving or operating machinery could be risky too. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the piece worth knowing: the codeine in CoActifed is a prodrug. Your liver enzyme CYP2D6 has to convert it into morphine before you get any real pain relief or cough benefit. In a test tube, kava looked like it might block CYP2D6, which could mean less codeine turning into morphine and a weaker effect. But the good news is that actual human studies didn't show kava meaningfully changing how this enzyme works, so this is rated minor and unlikely.

So in practice, I wouldn't expect a problem here. My bigger caution is overlap: kava is calming and codeine is sedating, so combining them could leave you extra drowsy. Kava can also rarely stress the liver, so it's worth running this combo past your pharmacist or doctor first.
