CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Fluticasone Propionate
Fluticasone Propionate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: this pairing is pretty low on my worry list. Fluticasone propionate is processed by a liver enzyme called CYP3A4, and the concern is whether kava slows that enzyme down. If it did, more fluticasone could linger in your body, which in theory might raise the chance of steroid-related effects. With nasal sprays like Flonase or Xhance, that mostly means local stuff (nasal dryness, irritation, or nosebleeds), since very little is absorbed.
But the better-quality human research found kava had no real effect on CYP3A4, so a meaningful interaction is unlikely. My bigger caution with kava is its rare risk of liver harm. Check with your pharmacist or doctor before combining them, especially if your liver isn't perfectly healthy.
