Topiramate falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the thing to keep in mind: kava has a calming, sedative effect on the brain, and so does topiramate. When you put the two together, those effects can stack on top of each other. That means more drowsiness, dizziness, trouble concentrating, slowed thinking, and clumsy or slowed reflexes than you'd expect from either one alone.

This isn't about kava changing how your body breaks topiramate down. It's simply two things pulling in the same direction at the same time. Topiramate already tends to cause grogginess and mental fog for many people, so adding kava can make that noticeably worse and may affect driving. There's also a separate concern that both kava and topiramate can stress the liver. Please talk with your pharmacist or doctor before combining them.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

What Is Topiramate Used For?

Brand names: Topamax, Trokendi XR, Qudexy XR, Eprontia

  • Topiramate is used alone or with other medications to treat certain types of seizures including primary generalized tonic-clonic seizures (formerly known as a grand mal seizure; seizure that involves the entire body) and partial onset seizures (seizures that involve only one part of the brain). Topiramate is also used with other medications to control seizures in people who have Lennox-Gastaut syndrome (a disorder that causes seizures and developmental delays). Topiramate is also used to prevent migraine headaches but not to relieve the pain of migraine headaches when they occur. Topiramate is in a class of medications called anticonvulsants. It works by decreasing abnormal excitement in the brain.
