Why this applies to Flutamide

Flutamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture for flutamide and kava. Your liver uses an enzyme called CYP1A2 to help process flutamide. The thought was that kava might slow that enzyme down, which could let a bit more flutamide build up. In theory that might mean stronger drug effects or more side effects like nausea, diarrhea, or hot flashes.

But the real-world evidence doesn't back that up. In a careful human study, kava had no meaningful effect on CYP1A2, so this interaction is unlikely to cause you trouble. The bigger concern is actually liver safety: flutamide can already stress the liver, and kava has been tied to rare liver damage. Because of that overlap, please check with your oncologist or pharmacist before combining them.

Here's the short version on combining kava with flutamide: the worry on paper is that kava might block an enzyme called CYP3A4, which is one of the pathways your body uses to process flutamide. If kava slowed that enzyme down, flutamide could build up a bit, possibly nudging up side effects like nausea, diarrhea, or hot flashes.

The good news is that the better-quality human research doesn't support this. In a 28-day study, kava didn't actually change CYP3A4 activity, so a real-world interaction is unlikely. My bigger concern is something else entirely: both kava and flutamide can stress the liver, so stacking them together is risky. Please check with your oncology team or pharmacist before adding kava.

What Is Flutamide Used For?

Brand names: Eulexin, Euflex

Eulexin (flutamide) capsules are used to treat prostate cancer in men. They are always used together with an LHRH agonist (a hormone-lowering injection), never alone.

Eulexin is indicated for two stages of prostate cancer: locally confined Stage B2-C prostate cancer (used alongside goserelin acetate and radiation therapy, starting eight weeks before radiation begins) and Stage D2 metastatic prostate cancer (cancer that has spread beyond the prostate, taken with an LHRH agonist until the disease progresses).
