Adrafinil + Flutamide
Flutamide brand names: Eulexin, Euflex.
Flutamide falls into 2 drug categories that Adrafinil can interact with; the most serious is rated moderate. Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Adrafinil interacts with whole categories of medication. Flutamide falls into 2 of those categories, so it can interact with Adrafinil in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Flutamide
Flutamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for flutamide. Your liver uses an enzyme called CYP1A2 to process this medicine. Adrafinil (which your body turns into modafinil) may slightly speed that enzyme up. In theory, that could clear flutamide a little faster and leave a bit less of it in your system, which might weaken how well it blocks androgen and helps control your prostate cancer.
I want to be honest about how soft this is. It's a theoretical concern based only on lab data, and the enzyme effect is described as weak. Still, since you're treating cancer, you don't want anything quietly chipping away at your therapy. Please don't start adrafinil without first running it by your oncologist or pharmacist.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Flutamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with flutamide and adrafinil. Your body turns adrafinil into modafinil, and modafinil can mildly speed up a liver enzyme called CYP3A4. Flutamide leans on that same enzyme to be processed. So in theory, adrafinil could nudge your body to clear flutamide a little faster, which might slightly weaken how well it blocks male hormones to fight your prostate cancer.
I want to be honest about the strength of this: it's theoretical, based on how the drugs work rather than real patient studies, and modafinil is only a weak enzyme booster. Still, since flutamide is part of your cancer treatment, this isn't something to guess about. Please run it by your oncologist or pharmacist before mixing the two.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
- AHigh-quality meta-analysis (quantitative systematic review).
- BCase-control study.
- CConsensus.
- DAnecdotal evidence.

From our pharmacist’s overview on the Flutamide page
“A man taking flutamide (Eulexin) to block testosterone during prostate cancer treatment cannot afford big swings in the drug's level, and most of the 189 supplements on this page are capable of causing exactly that. The bulk of them work through the liver enzyme CYP3A4, which breaks flutamide down.”

About Flutamide
What Is Flutamide Used For?
Brand names: Eulexin, Euflex
Eulexin (flutamide) capsules are used to treat prostate cancer in men. They are always used together with an LHRH agonist (a hormone-lowering injection), never alone.
Eulexin is indicated for two stages of prostate cancer: locally confined Stage B2-C prostate cancer (used alongside goserelin acetate and radiation therapy, starting eight weeks before radiation begins) and Stage D2 metastatic prostate cancer (cancer that has spread beyond the prostate, taken with an LHRH agonist until the disease progresses).
