Medroxyprogesterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body uses an enzyme called CYP3A4 to help clear medroxyprogesterone (Provera, Depo-Provera). Early lab tests hinted that kava might slow that enzyme down, which in theory could let a little more of the hormone hang around. If that happened, you might notice more of the usual progestin effects, like breast tenderness, bloating, mood changes, or spotting.

But the better-quality human research is reassuring. In a real study, people taking kava daily for a month showed no meaningful change in CYP3A4 activity. So this interaction is considered minor and unlikely to cause trouble. The bigger thing I'd watch with kava is its rare link to liver problems, so do check in with your pharmacist or doctor before starting it.

What Is Medroxyprogesterone Used For?

Brand names: Provera, Amen, Depo-Provera, Depo-subQ Provera 104

Medroxyprogesterone is used for different conditions depending on the product form. The injectable forms — including Depo-Provera and intramuscular medroxyprogesterone acetate injectable suspension — are used to prevent pregnancy. Depo-subQ Provera 104 (the subcutaneous injection) is also approved to manage pain associated with endometriosis.

The oral tablets — sold under the brand Provera and as generic medroxyprogesterone acetate tablets — treat secondary amenorrhea (periods that have stopped) and abnormal uterine bleeding caused by hormonal imbalance. The tablets are also used to prevent endometrial hyperplasia (abnormal thickening of the uterine lining) in postmenopausal women with a uterus who are taking daily conjugated estrogens.
