CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Lidocaine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kava interacts with.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the honest picture with kava and lidocaine. In a test tube, kava extract slows down an enzyme called CYP2D6, which is one of the pathways your body uses to clear lidocaine. On paper, slowing that enzyme could let lidocaine build up a bit, which might mean more of its effects or side effects like dizziness, drowsiness, or numbness or tingling around the mouth. Lidocaine is not a prodrug, so it doesn't need that enzyme to start working.
The good news: actual studies in people show kava does not really change how CYP2D6 medications are handled, so this is mostly theoretical. Most lidocaine you'd use is a cream or patch, and very little gets into your bloodstream. Still, it's worth a quick word with your pharmacist if you're using kava regularly.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Lidocaine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with lidocaine: there isn't much to worry about. The theory is that lidocaine is partly cleared by a liver enzyme called CYP3A4, and some early lab (test tube) studies hinted that kava might slow that enzyme down. If that were true, lidocaine could build up a bit. But better human research tells a calmer story. In one study, healthy volunteers took kava for 28 days and it did not change CYP3A4 activity at all.
Practically speaking, most folks use lidocaine as a patch or cream for nerve or muscle pain, so very little reaches the bloodstream anyway. The bigger thing I'd flag is kava's rare link to liver damage. Always check with your pharmacist or doctor before adding kava.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. What Is Lidocaine Transdermal Patch Used For?
Brand names: Xylocaine, Dilocaine, Lidoject-1, Xylocaine MPF, Nervocaine, Lidoject-2, Duo-Trach Kit, Octocaine, Lidoderm, Ztlido
Lidocaine skin patches are used in adults to relieve pain from post-herpetic neuralgia (PHN). This is nerve pain that can linger after shingles.
Bondlido (lidocaine topical system 10%) is specifically labeled for this use. Ztlido (lidocaine topical system 1.8%) is also covered by label directions for treating PHN.
