P-GLYCOPROTEIN SUBSTRATES

Conjugated Estrogens falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on combining kava with Premarin (conjugated estrogens). Estrogens are partly moved around the body by a transporter called P-glycoprotein, and lab dish studies hint that kava might slow that transporter down. In theory, that could mean a little more estrogen hanging around, which might nudge up side effects like breast tenderness, bloating, nausea, or headaches.
The honest truth is this is weak, theoretical evidence. When researchers actually tested kava in people using another P-gp drug, they saw no real change. So a meaningful interaction here is unlikely. My bigger concern is kava's own rare risk of liver harm. Please loop in your pharmacist or doctor before pairing these two, especially if your liver isn't in great shape.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Conjugated Estrogens also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body uses an enzyme called CYP3A4 to help break down conjugated estrogens like Premarin. Some early lab studies hinted that kava might slow that enzyme down, which in theory could let estrogen build up a bit. But the better human research tells a calmer story. In a study where healthy volunteers took kava for 28 days, it didn't actually change CYP3A4 activity.
So for most people this combination is unlikely to cause a real problem, and it's rated as a minor concern. That said, kava has been tied to rare but serious liver injury, and your liver also processes estrogen, so I'd still loop in your pharmacist or doctor before pairing them, especially if you have any liver concerns.
