Almotriptan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with almotriptan (Axert): your body clears almotriptan partly through a liver enzyme called CYP3A4. Early lab-dish studies hinted that kava might slow this enzyme down, which could in theory let almotriptan build up and bring on more of its usual effects, like dizziness, drowsiness, flushing, or a tight feeling in the chest or throat. But the better human research tells a calmer story. In a real-world study, healthy people took kava daily for a month and it did not change CYP3A4 activity.

So this interaction is rated minor and unlikely. The bigger thing I'd flag is kava's own rare risk of liver harm, and that both kava and almotriptan can make you feel sleepy. Please run this combo by your pharmacist or doctor before mixing them.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. About Almotriptan

What Is Almotriptan Used For?

Brand names: Axert, Almogran

Almotriptan tablets are used to treat migraine headache attacks — with or without aura — as they happen. They are not for preventing migraines before they start.

In adults, almotriptan tablets treat acute migraine attacks. In adolescents ages 12 to 17, almotriptan tablets treat acute migraine headache pain in those who typically have attacks lasting 4 hours or more when untreated. Note that in adolescents, the effect on migraine-related symptoms such as nausea and light sensitivity was not established in clinical trials. Almotriptan is not indicated for cluster headache or for hemiplegic or basilar migraine.
