Why this applies to Naproxen, Pseudoephedrine

Naproxen, Pseudoephedrine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C9 substrates.
In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.

Here's the simple version. The naproxen part of Aleve Cold and Sinus is cleared from your body partly by a liver enzyme called CYP2C9. In lab dish studies, kava appears to slow that enzyme down. If that same effect happened in people, naproxen could hang around longer and build up a bit, which might mean more of its usual side effects like stomach upset, heartburn, or a higher risk of stomach irritation or bleeding. The pseudoephedrine (the decongestant) isn't really involved in this particular interaction.

I want to be honest about the strength of this. This has only been seen in test tubes, not in actual humans. So it's more of a theoretical caution than a proven problem. Still, since kava itself can be hard on the liver, I'd run it by your pharmacist or doctor before combining the two.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture with Aleve Cold and Sinus (naproxen plus pseudoephedrine) and kava. The theory is that kava might slow down a liver enzyme called CYP1A2, which could let some drugs build up. But the better-quality human research doesn't back that up. In one study, healthy people took a real kava dose for 28 days and saw no change in CYP1A2 activity. So this concern is mostly lab-dish and case-report level, not something seen reliably in people.

Practically, I wouldn't expect kava to meaningfully boost your naproxen or pseudoephedrine here. A bigger thing to keep in mind is that kava has been linked to rare liver injury, and naproxen can be hard on the stomach, so check with your pharmacist before pairing them.
