Metoprolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the gist for metoprolol and kava. Your body clears metoprolol using a liver enzyme called CYP2D6. In a test tube, kava appears to block that enzyme, which in theory could let metoprolol build up and slow your heart rate or drop your blood pressure too much (think dizziness, fatigue, or feeling lightheaded). But here's the reassuring part: when this was actually studied in people, kava did not meaningfully change how CYP2D6 drugs are handled.

So this is a minor, unlikely interaction based on weak evidence. The bigger thing I'd flag is kava's rare risk of liver harm, which matters for anyone on regular medication. Please check with your pharmacist or doctor before combining them.

About Metoprolol

What Is Metoprolol Used For?

Brand names: Lopressor, Toprol XL

Metoprolol is used to treat several heart and blood pressure conditions. Metoprolol succinate extended-release tablets (including Toprol-XL) are approved for high blood pressure, angina pectoris (chest pain from the heart), and heart failure. Metoprolol tartrate tablets (including Lopressor) are approved for high blood pressure, angina, and — when used alongside intravenous metoprolol — to reduce the risk of cardiovascular death after a definite or suspected heart attack.

Metoprolol tartrate injection (intravenous form) is used in hemodynamically stable patients with a definite or suspected acute heart attack to reduce cardiovascular death, and is followed by oral metoprolol therapy. Lowering blood pressure with metoprolol reduces the risk of strokes and heart attacks.
