Why this applies to Escitalopram

Escitalopram falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the situation with kava and escitalopram (Lexapro). Your body relies partly on a liver enzyme called CYP2C19 to break escitalopram down. In test-tube studies, kava slows that enzyme. If that happened in people, escitalopram could clear out more slowly and build up a bit, which might mean stronger effects or more side effects like drowsiness, nausea, or feeling jittery.

I'll be honest, though: this is only lab-based so far and hasn't actually been seen in real patients, so it's more theoretical than proven. The bigger practical worry is that both kava and escitalopram can feel sedating and both can affect the liver. Please loop in your pharmacist or doctor before pairing them so they can look at your full picture.

Escitalopram also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on this pairing: the enzyme question you're asking about (whether kava slows down CYP3A4, one of the liver pathways that helps clear escitalopram) looks like a non-issue. Early lab-dish studies hinted kava might block that enzyme, but a better human study where volunteers took 1000 mg of kava twice daily for a month found no real effect on CYP3A4. So kava is unlikely to make your Lexapro build up.

That said, I'd still flag a different, more practical concern: both kava and escitalopram have calming effects, so together they could leave you feeling extra drowsy, foggy, or tired. Kava has also been linked to rare liver damage. Please check with your pharmacist or doctor before combining them.

Escitalopram treats major depressive disorder (MDD) in adults and in teens 12 and older. It also treats generalized anxiety disorder (GAD), which means excessive, hard-to-control worry, in adults.

Some details depend on the product. Lexapro tablets are also labeled for GAD in children 7 and older. Escitalopram capsules are labeled for MDD and GAD in adults younger than 65. The capsules are also labeled for MDD in teens 12 and older.
