Why this applies to Clozapine

Clozapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the basic idea with these two together. Your body clears clozapine mostly through a liver enzyme called CYP1A2. There was some early lab and case-report concern that kava might slow that enzyme down, which in theory could let clozapine build up and bring on more side effects like heavy drowsiness, dizziness, drooling, low blood pressure, or constipation.

The good news is that the better human evidence doesn't back that up. In a controlled study, a real-world kava dose didn't change CYP1A2 activity at all, so any actual interaction looks unlikely and minor. I'd still flag two things: clozapine is a serious medicine that needs close monitoring, and kava has been tied to rare liver harm. Please check with your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the gist for Clozapine and kava. The worry on paper is that kava might block an enzyme called CYP2D6, which is one of the helpers your liver uses to clear clozapine. If that enzyme slowed down, clozapine could theoretically build up and bring more side effects like heavy drowsiness, dizziness, low blood pressure, or constipation. The good news is that while lab dish studies showed this blocking effect, actual human studies did not find kava changing how the body handles CYP2D6 drugs. So this interaction is rated minor and unlikely.

The bigger concern is that both kava and clozapine can stress the liver and both can make you sleepy. Because clozapine needs careful monitoring, please loop in your pharmacist or doctor before adding kava.

What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
