Henbane and Clozapine

Here is the interaction we found between these two, with the evidence behind it.

Major

ANTICHOLINERGIC DRUGS

Why this applies to Clozapine

Clozapine falls under ANTICHOLINERGIC DRUGS — the group Henbane interacts with.

Concomitant use of henbane can have additive anticholinergic effects and adverse effects with amantadine, antihistamines, atropine, belladonna alkaloids, hyoscyamine, phenothiazines, procainamide, scopolamine, and tricyclic antidepressants.
Severity Major Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing with henbane and clozapine: clozapine already has strong anticholinergic activity on its own, meaning it blocks that same acetylcholine messenger that henbane's alkaloids block. Stack them together and those effects can pile up. That can show up as dry mouth, blurry vision, constipation (clozapine is notorious for this and it can become dangerous), trouble urinating, a racing heart, confusion, and in severe cases overheating or delirium.

This warning is based on how these chemicals behave (pharmacology), not on a big pile of patient studies, so it's more of a logical caution than a proven event. Still, I'd take it seriously. Honestly, henbane isn't safe to use as a supplement at all. Please loop in your doctor or pharmacist before mixing anything like this with clozapine.

From our pharmacist’s overview on the Clozapine page

“Clozapine (Clozaril, Versacloz) is usually the last-resort antipsychotic for schizophrenia, prescribed under close monitoring because small shifts in its blood level matter, so the 212 supplement interactions on file here deserve genuine care rather than a quick skim. Six are rated major.”

About Henbane

What is Henbane?

Henbane
  • Pain relief
  • Muscle spasms and cramps
  • Sleep and sedation
  • Asthma and breathing problems (traditional)
  • Toothache (traditional)

Henbane is a highly poisonous plant containing potent alkaloids like hyoscyamine and scopolamine, and even small amounts can cause serious or deadly poisoning. While its compounds have real medical uses, these belong in carefully controlled prescription drugs, not in self-made or self-dosed herbal products. We strongly advise against using henbane as a supplement.

About Clozapine

What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
