Nimodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the gist for nimodipine and kava. Nimodipine is handled by a liver enzyme called CYP3A4, and the worry on paper is that kava might slow that enzyme down. If that happened, nimodipine could build up and lower your blood pressure too much, leaving you dizzy, lightheaded, or flushed. The good news is the evidence doesn't really back that up. While lab dish studies hinted kava might block CYP3A4, a real human study found kava had no meaningful effect on this enzyme.

So this is a minor, unlikely interaction. That said, nimodipine is usually given in a serious hospital setting after a brain bleed, and kava carries its own liver risks, so please check with your doctor or pharmacist before combining them.

What Is Nimodipine Used For?

Nimodipine — available as capsules and oral solution — is used to improve neurological outcomes after a subarachnoid hemorrhage (SAH), a type of bleeding that occurs around the brain when an intracranial berry aneurysm ruptures. It works to reduce the incidence and severity of ischemic deficits (areas of the brain damaged by reduced blood flow) that can follow this kind of bleed.

This use applies across nimodipine capsules, nimodipine oral solution, and Nymalize oral solution. It is indicated for adult patients regardless of how severe their neurological condition is at the time treatment begins.
