Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the simple version: both kava and your Ingrezza can slow down your central nervous system, and stacking them can add up. Valbenazine already tends to cause drowsiness and sometimes fatigue, balance trouble, or feeling a bit out of it. Kava has its own calming, sedating effect, so taking the two together may leave you more sleepy, slower to react, and less steady on your feet than either one alone.

This is a well-supported, meaningful interaction, and guideline groups suggest not mixing kava with other sedating medicines. There's also kava's separate risk of liver injury to keep in mind. Please don't change anything on your own. Just check with your pharmacist or doctor before pairing these two.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the gist for Ingrezza (valbenazine) and kava. Your body uses an enzyme called CYP2D6 to help break down part of valbenazine's active form. In a test tube, kava extract looked like it could block that enzyme, which in theory might let a bit more drug linger and nudge up side effects like drowsiness, dizziness, or trouble with balance. The reassuring part: when this was actually studied in real people, kava did not meaningfully change how CYP2D6 drugs were handled.

So this is a minor, unlikely interaction based on lab work that didn't pan out in humans. My bigger concern with kava is its rare link to liver damage, so loop in your pharmacist or doctor before combining the two.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with Ingrezza (valbenazine). Valbenazine relies on a liver enzyme called CYP3A4 to be processed, and on paper kava was once suspected of slowing that enzyme down. If that were true, valbenazine could build up and you might feel more of its side effects, like sleepiness, balance trouble, or a slower heartbeat.

The good news: the better human research doesn't back this up. A real-world study in healthy people found that kava had no meaningful effect on CYP3A4, so this interaction is rated minor and unlikely. A bigger concern is that both kava and valbenazine can stress the liver, so it's worth running this combo by your pharmacist or doctor first.

What Is Valbenazine Used For?

Brand name: Ingrezza

Valbenazine — sold as Ingrezza and INGREZZA Sprinkle — is approved for two conditions in adults. The first is tardive dyskinesia, a movement disorder that causes uncontrolled, repetitive facial or body movements, often brought on by long-term use of certain psychiatric medications. The second is chorea associated with Huntington's disease, which refers to the involuntary, dance-like jerking movements that are a hallmark of that condition.

Both Ingrezza and INGREZZA Sprinkle are approved for both of these uses.
