Why this applies to Apixaban

Apixaban falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the situation with kava and your Eliquis (apixaban). Apixaban gets pumped out of your body partly by a transporter called P-glycoprotein (P-gp). In a lab dish, kava seemed to slow that pump down. If that actually happened in people, less apixaban would be cleared, so more could build up. With a blood thinner, that could mean a higher chance of bruising or bleeding, like nosebleeds, bleeding gums, or harder-to-stop cuts.

The good news: this is mostly test-tube evidence. A human study using a similar pump-cleared drug (digoxin) showed no real change. So the worry is theoretical, and higher kava doses are the bigger question mark. I'd still loop in your pharmacist or doctor before combining them, partly because kava can also stress the liver.

Apixaban also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the honest picture with Eliquis (apixaban) and kava. Apixaban is partly cleared by a liver enzyme called CYP3A4. In the test tube, kava looked like it might slow that enzyme down, which in theory could let apixaban build up and raise bleeding risk (think easy bruising, nosebleeds, or bleeding that's slow to stop). But here's the reassuring part: in an actual human study, kava taken for 28 days had no real effect on CYP3A4. So this concern hasn't panned out in people.

My bigger worry is separate: kava has been tied to rare liver injury, and a stressed liver can change how you handle apixaban. Please loop in your pharmacist or doctor before combining them.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
