Bosutinib falls under HEPATOTOXIC DRUGS — the group Kava interacts with.

Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the thing to know about pairing kava with Bosutinib (Bosulif). This isn't about kava changing how your body breaks down the medication. Instead, both substances can be hard on the liver, and stacking two liver stressors together may raise your chances of liver trouble. Bosutinib already carries a known risk of liver enzyme elevations, and your cancer team likely checks your liver blood work regularly while you're on it.

To be fair, this concern is mostly theoretical and based on scattered case reports, not strong studies. Most serious kava liver cases involved heavy, long-term use. Still, with a medication this important, I'd play it safe. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your oncologist or pharmacist before using kava.

Bosutinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with Bosulif (bosutinib). Your body clears bosutinib mainly through a liver enzyme called CYP3A4. The theory was that kava might block that enzyme, which could let bosutinib build up and bring on more of its usual side effects, like diarrhea, nausea, stomach pain, low blood counts, or liver irritation. But the better human research doesn't back that up. In one study, healthy people took kava for 28 days and it didn't meaningfully change CYP3A4 activity.

So this particular interaction is rated minor and unlikely. The bigger concern here isn't enzymes at all, it's that both kava and bosutinib can stress the liver. Please loop in your oncology team or pharmacist before adding kava.

What Is Bosutinib Used For?

Brand name: Bosulif

Bosutinib is used to treat Philadelphia chromosome-positive chronic myelogenous leukemia (Ph+ CML) — a type of blood cancer. It can be used for newly diagnosed patients or for those whose cancer has stopped responding to, or who cannot tolerate, a prior treatment.

Bosulif (the brand-name product) is approved for adults and children 1 year and older with chronic phase Ph+ CML, and for adults with accelerated phase or blast phase Ph+ CML that is resistant or intolerant to prior therapy. Generic bosutinib tablets are approved for adults only with the same range of disease phases.
