Estradiol, Norethindrone Acetate falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist for your Activella (estradiol with norethindrone). Kava might, in theory, block a cellular pump called P-glycoprotein that helps move some medications through the body. If that pump slows down, a little more of your hormone medicine could stay in circulation. In plain terms, that might nudge up the usual estrogen-type effects, like breast tenderness, bloating, nausea, or headaches.

But honestly, the evidence here is thin. This idea comes mostly from lab dish studies, and when researchers actually tested kava in people with a similar pump-dependent drug, they saw no real change. So this is more of a 'keep an eye on it' note than an alarm. Bigger concern with kava is its rare link to liver problems. Please run it past your pharmacist or doctor before combining the two.

In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the short version on combining kava with Activella (estradiol and norethindrone): the theoretical concern is that kava might block a liver enzyme called CYP1A2, which helps process estrogen. If that happened, estrogen levels could creep up a bit, which in theory might mean more side effects like breast tenderness, bloating, nausea, or headaches.

But honestly, the evidence here is weak and mixed. A careful clinical study in healthy people found that kava had no real effect on this enzyme. So this interaction is rated minor and unlikely. The bigger thing worth knowing is that kava itself has been tied to rare liver damage, so it's smart to run it by your pharmacist or doctor before starting it.

Your Activella combines two active hormones, estradiol and norethindrone, and both are processed by a liver enzyme called CYP3A4. The thought here is that kava might slow that enzyme down, which could theoretically let a bit more hormone linger in your body and slightly bump up side effects like breast tenderness, bloating, nausea, or breakthrough spotting.
