Here's the gist for your Activella (estradiol plus norethindrone). Adrafinil turns into modafinil in your body, and modafinil can mildly speed up a liver enzyme called CYP1A2. That same enzyme helps process the estradiol in your hormone tablet. So in theory, taking adrafinil alongside it could clear the estrogen a little faster and make your hormone therapy a touch less effective.

I want to be honest about how shaky this is. This is only a theoretical concern from lab studies, and the effect is weak. If it mattered at all, you might notice symptoms creeping back, like hot flashes, night sweats, or breakthrough spotting. I wouldn't lose sleep over it, but please loop in your doctor or pharmacist before pairing these, especially since adrafinil is unregulated and hard on the liver.
