Cobimetinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with cobimetinib (Cotellic). Cobimetinib is cleared from your body by a liver enzyme called CYP3A4. The thought is that if kava slowed that enzyme down, cobimetinib could build up and cause more side effects like rash, diarrhea, fatigue, or vision changes. In theory.

But the actual evidence here is reassuring. Early lab (test tube) studies hinted kava might block CYP3A4, yet a better human study showed kava had no real effect on this enzyme. So this interaction is rated minor and unlikely. My bigger concern is that both kava and cancer therapy can stress the liver, so don't pair them without checking first. Please run it by your oncologist or pharmacist before adding kava.

About Cobimetinib

What Is Cobimetinib Used For?

Brand name: Cotellic

Cobimetinib is approved for two uses in adults. First, it is used in combination with another drug called vemurafenib to treat melanoma (a type of skin cancer) that cannot be surgically removed or has spread to other parts of the body — but only when the tumor carries a specific genetic change called a BRAF V600E or V600K mutation. A lab test must confirm this mutation before treatment begins.

Second, cobimetinib can be used on its own (as a single agent) to treat adults with histiocytic neoplasms — a rare group of cancers involving immune cells called histiocytes.
