Lansoprazole, Amoxicillin, Clarithromycin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

This combo pack (Prevpac) treats stomach ulcers tied to H. pylori. The piece that matters here is lansoprazole, the acid-reducing part, which your body partly clears through a liver enzyme called CYP2C19. In a test-tube setting, kava appears to slow that enzyme down. If that happened in real life, lansoprazole could build up a bit more than usual, which might mean slightly stronger effects or side effects like headache, nausea, or diarrhea.

I'll be honest with you though: this is only lab-based research and has not been seen in actual people, so it's more of a theoretical caution than a proven problem. There's also a separate, more important reason to be careful: kava has been linked to rare liver injury. Please check with your pharmacist or doctor before pairing the two.

Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Prevpac is a short combo pack (lansoprazole plus the antibiotics amoxicillin and clarithromycin) used to knock out an H. pylori stomach infection. Here's the thing to keep in mind: both kava and a few of these medicines, especially clarithromycin, can put a bit of stress on your liver. Stacking them together may add up and raise the odds of liver trouble.

To be fair, this is a theoretical concern based mostly on scattered case reports, and most serious kava liver problems happened with heavy, long-term use. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. The Prevpac course is only about two weeks, so the simplest move is to pause kava during treatment. Please check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Lansoprazole, Amoxicillin, Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Prevpac is a combo pack of three medicines used to clear an H. pylori stomach infection: lansoprazole (an acid reducer), amoxicillin (a penicillin antibiotic), and clarithromycin. The concern on file is that clarithromycin is handled by a liver enzyme called CYP3A4, and lab tests once hinted that kava might block that enzyme, which in theory could let the drug build up.

The good news is that the better-quality human research doesn't back this up. In a study, people taking kava for 28 days showed no real change in CYP3A4 activity, so a meaningful interaction here is unlikely and minor. The bigger thing to know is that both kava and clarithromycin can stress the liver, so I'd run this past your pharmacist or doctor before combining them.
