Cevimeline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version on combining kava with cevimeline. Your body uses an enzyme called CYP2D6 to help break down cevimeline. In a test tube, kava appears to slow that enzyme down, which in theory could let cevimeline build up a bit and make its usual effects stronger, things like extra saliva, sweating, watery eyes, an upset stomach, or feeling flushed.

The reassuring news is that when this was actually studied in people, kava did not meaningfully change how the body handles CYP2D6 medications. So this is mostly a lab-based concern, rated minor and unlikely. The bigger thing to keep in mind is kava's rare link to liver problems. It's always smart to run this combo by your pharmacist or doctor first.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the gist for cevimeline (Evoxac) and kava. Your body uses a liver enzyme called CYP3A4 to help break cevimeline down. Early lab-dish studies hinted that kava might slow this enzyme, which in theory could let cevimeline build up a bit and nudge up its usual effects, like extra sweating, watery eyes, runny nose, stomach upset, or feeling flushed.

The good news: a better human study found that kava actually had no real effect on CYP3A4. So this concern is mostly theoretical and rated minor and unlikely. The bigger thing I'd flag is that kava has been tied to rare liver injury, so it's worth a quick chat with your pharmacist or doctor before combining them.
