Methadone falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the thing with kava and methadone: both work to calm down your central nervous system. Methadone is a strong, long-acting opioid that already slows your breathing, makes you drowsy, and dulls your reflexes. Kava's relaxing compounds (kavalactones) push in the same direction, so stacking them can add up. That combination can leave you more sedated than expected, foggy, unsteady on your feet, and (most importantly) breathing too slowly or too shallowly.

This isn't a maybe. Good-quality evidence backs additive sedation, and expert guidelines specifically warn against mixing kava with sedating drugs. With methadone already so potent, please don't add kava without first talking to your pharmacist or doctor. One more note: both kava and methadone can stress the liver, which is another reason for caution.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the gist with kava and your methadone. On paper, kava can block an enzyme called CYP2D6, which is one of the pathways your body uses to clear certain drugs. In a test tube this looked significant, but when researchers actually studied it in people, kava did not meaningfully change how the body processed CYP2D6 drugs. So this particular concern is rated minor and unlikely.

That said, methadone is a potent opioid, and kava is a calming, sedating supplement. Even without a strong metabolism interaction, stacking two things that slow you down could add up to extra drowsiness, slowed breathing, or feeling foggy. Kava can also stress the liver in rare cases. Please loop in your pharmacist or doctor before combining them.

Methadone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on this pairing: while early lab tests hinted that kava might block CYP3A4 (one of the liver enzymes that helps clear methadone), better human studies didn't back that up. In healthy volunteers, kava had no real effect on this enzyme. So the worry that kava would cause methadone to pile up and amp up its effects looks unlikely based on the current evidence.

That said, there are two separate things I'd flag. Methadone is an active opioid, not a prodrug, and kava itself causes drowsiness and relaxation. Stacking two sedating things can leave you extra sleepy or foggy. Kava has also been tied to rare liver damage, which matters since methadone is hard on the liver too. Please loop in your pharmacist or doctor before combining them.

About Methadone

What Is Methadone Used For?

Brand name: Dolophine

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
