Enfortumab Vedotin-Ejfv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

If you're getting Padcev (enfortumab vedotin) infusions for bladder cancer, here's the honest picture on kava. Part of this drug is processed by a liver enzyme called CYP3A4. Old lab-dish studies hinted that kava might slow that enzyme, which in theory could let a bit more of the drug build up. But a better human study found kava had no real effect on CYP3A4 at all, so this concern is mostly theoretical and rated as minor and unlikely.

My bigger worry with kava is its rare but serious link to liver damage, especially while you're already on cancer treatment that stresses the body. Please loop in your oncology team or pharmacist before adding kava, so they can keep your liver labs in mind.

What Is Enfortumab vedotin-ejfv Injection Used For?

Brand name: Padcev

  • Enfortumab vedotin-ejfv injection is used to treat urothelial cancer (cancer of the lining of the bladder and other parts of the urinary tract) that has spread to nearby tissues or other parts of the body or cannot be removed by surgery, and has worsened after treatment with other chemotherapy medications or if these chemotherapy medications cannot be used for treatment. Enfortumab vedotin-ejfv injection is in a class of medications called monoclonal antibodies. It works by helping your immune system to slow or stop the growth of cancer cells.
