Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the honest scoop on combining kava with brexpiprazole (Rexulti). Your liver uses an enzyme called CYP2D6 to help break this medication down. In a test tube, kava extract slowed that enzyme, which in theory could let brexpiprazole build up and bring on more of its usual effects like drowsiness, dizziness, restlessness, or weight changes. Brexpiprazole works as-is (it's not a prodrug), so blocking the enzyme would mean stronger effects, not weaker ones.

The reassuring part: when researchers actually tested kava in real people, it did not meaningfully change how CYP2D6 drugs are processed. So this concern stays mostly theoretical and unlikely. My bigger flag is kava's rare link to liver damage. Please loop in your pharmacist or doctor before pairing these.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with brexpiprazole (Rexulti). Brexpiprazole is cleared from your body partly by a liver enzyme called CYP3A4. Some early lab studies hinted that kava might slow this enzyme down, which in theory could let brexpiprazole build up and cause more side effects like drowsiness, dizziness, restlessness, or weight gain. But here's the reassuring part: a better-quality human study found that kava actually had no real effect on CYP3A4. So this concern is mostly theoretical and unlikely to cause a problem.

My bigger caution isn't the enzyme at all. Both kava and brexpiprazole can make you feel sleepy or sedated, so together they may add up. Kava has also been tied to rare liver injury. Please check with your pharmacist or doctor before pairing them.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
