Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the thing to keep in mind: both kava and Perampanel (Fycompa) calm down activity in your brain. Perampanel already causes drowsiness, dizziness, and trouble with balance or coordination as some of its most common side effects, and it even carries a warning about serious mood and behavior changes. Adding kava, which has its own sedating, GABA-like effects, can stack on top of that.

The likely result is feeling extra sleepy, unsteady, or slowed down, which raises your risk of falls or feeling foggy. There's also a real liver concern, since kava has been tied to rare liver damage. I wouldn't pair these without checking first. Please talk with your doctor or pharmacist before using kava with Perampanel.

Here's the gist for your situation. Perampanel (Fycompa) is partly cleared by a liver enzyme called CYP2C19. Lab studies suggest kava can slow that enzyme down. If that played out in the body, your perampanel could clear a little slower and build up, which might nudge up its calming side effects like drowsiness, dizziness, trouble walking, or feeling irritable or foggy.

That said, this is theoretical. It's only been seen in test-tube research, not in real people, so we don't know how meaningful it is. There's a bigger worry too: both kava and perampanel can stress the brain (extra sleepiness) and kava has been tied to rare liver harm. Please don't add kava without first checking with your pharmacist or doctor.

Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture with kava and perampanel (Fycompa). The theory is that kava might slow down a liver enzyme called CYP1A2, which helps your body process perampanel. If that happened, perampanel could build up a bit and you might feel more of its usual effects, like drowsiness, dizziness, trouble with balance, or irritability and mood changes.

But the research here is shaky and conflicting. A good-quality study in healthy people found kava had no real effect on this enzyme, so a meaningful interaction is unlikely. What I'd worry about more is that both kava and perampanel can be hard on the body, kava on the liver and perampanel causing sleepiness, so the calming effects could stack. Please run this by your pharmacist or doctor before combining them.

The theory was that kava might slow that enzyme, which could let perampanel build up and cause more of its usual effects, like sleepiness, dizziness, trouble walking, or irritability. But honestly, the evidence here is reassuring. A clinical study in healthy people taking kava for a month showed no real effect on CYP3A4, so this interaction is rated minor and unlikely.

The bigger thing I'd flag isn't this enzyme question at all. Both kava and perampanel can make you drowsy, and kava has been linked to rare liver damage. So before combining them, please check with your pharmacist or doctor.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
