Dextromethorphan HBr falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version: the cough suppressant in Vicks Formula 44, dextromethorphan, is cleared from your body mainly by a liver enzyme called CYP2D6. In a test tube, kava looked like it could slow that enzyme down, which in theory might let dextromethorphan linger longer and bump up its usual effects like drowsiness or dizziness.

But here's the reassuring part: when researchers actually studied this in real people, kava did not meaningfully change how the body handles CYP2D6 drugs. So this is mostly a lab-bench concern, rated minor and unlikely. I'd still mention it to your pharmacist, mainly because both kava and dextromethorphan can make you sleepy, and combining them could add to that drowsiness.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Good news here: this pairing is pretty low on the worry list. The concern on file is that kava might slow down an enzyme called CYP3A4, which is one of the pathways your body uses to clear dextromethorphan. If that happened, the cough medicine could linger a bit longer and you might feel more drowsy, dizzy, or foggy than usual.

But the better-quality human research doesn't back that up. In one study, people took kava daily for 28 days and it did not meaningfully change CYP3A4 activity. So in real life, a regular dose of cough syrup alongside kava is unlikely to cause a problem. The bigger thing I'd flag is that both kava and dextromethorphan can make you sleepy, so combined drowsiness is the more realistic issue. Check with your pharmacist if you take either one often.
