Dextromethorphan HBr falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.

Theoretically, grape seed extract may increase the levels of CYP2D6 substrates.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.

Here's the gist for your cough medicine. Your body relies on a liver enzyme called CYP2D6 to clear dextromethorphan. In lab dish studies, grape seed extract seems to slow that same enzyme down a bit. If that held true in people, the dextromethorphan could linger longer and feel stronger, which might mean more drowsiness, dizziness, or, at higher levels, feeling foggy or restless.

The important caveat: this is only test-tube evidence and has not actually been seen in real patients. Eating grapes is no concern at all. Concentrated grape seed supplements are the only theoretical question mark. If you take one regularly and use dextromethorphan often, just run it by your pharmacist to be safe.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the honest picture with grape seed extract and your cough medicine. Dextromethorphan is partly cleared by a liver enzyme called CYP3A4, and grape compounds might nudge that enzyme. The tricky part is that the research pulls in two directions: some lab studies suggest grape seed extract slows this enzyme down, while animal studies suggest it might speed it up. We honestly can't say which way it goes in people.

If it slowed the enzyme, dextromethorphan could build up a bit and cause more drowsiness or dizziness. If it sped it up, your cough relief might wear off sooner. Right now this is only theoretical lab and animal data, never reported in humans, and the small amount in foods or normal cough syrup use is unlikely to cause trouble. Still, run concentrated grape seed supplements by your pharmacist first.
