CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Estradiol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with grape and your estradiol. One small study found that grape juice may speed up an enzyme in your body called CYP1A2, which is one of the pathways that helps process estradiol. If that enzyme works a little faster, your body could clear estradiol a bit quicker, which in theory might make it slightly less effective. For vaginal estradiol products, that could mean your dryness, itching, or hot flashes don't ease up as much as expected.
I want to be honest though: this is mostly theoretical and based on weak evidence, and it was studied with grape juice, not concentrated grape supplements. Everyday grapes and juice are very unlikely to cause a problem. If you're considering a grape seed extract supplement, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Estradiol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body relies on a liver enzyme called CYP3A4 to break down estradiol. Grape seed extract might nudge that enzyme, but honestly the science is muddy. Lab (test tube) studies hint it could slow the enzyme down, which would let more estradiol linger and possibly bump up estrogen-type effects like breast tenderness, bloating, nausea, or spotting. Animal studies suggest the opposite, that it speeds the enzyme up and clears estradiol faster, which could mean less relief from symptoms like vaginal dryness or hot flashes.
The reassuring part: this has not actually been seen in people, and it's based on early lab and animal work only. Eating grapes is not a concern. If you're using a concentrated grape seed supplement, just give your pharmacist or doctor a heads-up so they can keep an eye on things.
