CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for grape and your verapamil. Some research suggests grape juice may speed up an enzyme called CYP1A2. If a medication is cleared by that enzyme, revving it up could lower the drug's levels, potentially making it work a little less well. In theory, that might mean weaker blood pressure control, more chest pain, or less steady heart rate control from your verapamil.
That said, I'd keep this in perspective. The evidence here is thin and mostly theoretical, and verapamil is actually handled mainly by a different pathway (CYP3A4), so CYP1A2 isn't its main route. Enjoying grapes as food is very unlikely to matter. If you're thinking about a concentrated grape supplement, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Verapamil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the honest picture with grape seed extract and verapamil. Your body clears verapamil using a liver enzyme called CYP3A4, and verapamil is the active drug itself (not a prodrug that needs switching on). So if grape seed extract were to slow that enzyme down, more verapamil could linger and you might notice things like dizziness, a slower heartbeat, low blood pressure, or constipation. If instead it sped the enzyme up, verapamil might clear faster and not control your blood pressure or heart rate as well.
The catch is that the research pulls in both directions, and so far this has only shown up in lab and animal studies, never in real people. Eating grapes is no concern. Before starting a concentrated grape seed supplement, just run it by your pharmacist or doctor.

What Is Verapamil Used For?
Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil
Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.
Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
