Why this applies to Bendamustine

Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.

Theoretically, grape juice might reduce the levels of CYP1A2 substrates.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.

Here's the gist for bendamustine (Treanda, Belrapzo, Vivimusta). The liver enzyme CYP1A2 helps process this chemotherapy drug. Some lab and small human studies suggest grape juice may slightly speed that enzyme up. In theory, a faster enzyme could clear bendamustine a bit quicker, which might mean a little less of it doing its cancer-fighting job. This is a theoretical concern only, based on weak evidence using a different test drug, so I don't want you to be alarmed.

Practically speaking, bendamustine is given by IV in a cancer clinic, where your team watches your dosing and bloodwork closely. Still, before adding any concentrated grape seed extract or grape supplement, run it by your oncology pharmacist or doctor. Enjoying actual grapes as food is unlikely to be a problem.

  • Chronic lymphocytic leukemia (CLL). How well it works compared with first-line treatments other than chlorambucil has not been established.
  • Indolent B-cell non-Hodgkin lymphoma (NHL) that has gotten worse during or within six months of treatment with rituximab or a rituximab-containing regimen.
