Ashitaba and Bendamustine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Bendamustine

Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashitaba interacts with.

In vitro research shows that ashitaba extract inhibits cytochrome P450 (CYP) 1A2. Theoretically, concomitant use of ashitaba with CYP1A2 substrates might decrease the clearance of these substrates and increase the risk for adverse effects. However, this interaction has yet to be reported in humans. Until more is known, use with caution.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Ashitaba and bendamustine. Bendamustine is partly handled by a liver enzyme called CYP1A2. In lab (test tube) studies, ashitaba appears to slow that same enzyme down. In theory, if your body clears bendamustine more slowly, more of it could linger and push up the chances of its usual side effects, things like low blood counts, fatigue, nausea, fever, and a higher infection risk.

I want to be honest about the strength here: this is theoretical and based only on lab work, not real cases in people. Still, bendamustine is a serious chemotherapy drug, so I wouldn't add a concentrated ashitaba supplement on your own. Please run it by your oncology team or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Bendamustine page

“Bendamustine (Treanda, Belrapzo, Vivimusta) is IV chemotherapy for chronic lymphocytic leukemia and certain lymphomas, and the liver clears it through an enzyme called CYP1A2. That enzyme is why nearly all 87 supplements on this page appear here, since about 80 of them can push CYP1A2 up or down, which could either raise side effects o…”

About Ashitaba

What is Ashitaba?

Ashitaba
  • General wellness and longevity
  • Antioxidant support
  • Heart and cholesterol health
  • Digestive and stomach support
  • Anti-inflammatory

Ashitaba is a leafy plant from Japan that is eaten as a vegetable and taken as a supplement for general health, antioxidant, and heart benefits. Most of the supporting research comes from laboratory and animal studies, so its real benefits in people are still uncertain. It appears generally well tolerated as a food, but talk to your pharmacist or doctor before using concentrated supplements.
