Why this applies to Bendamustine
Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this pairing. Once your body changes adrafinil into modafinil, that modafinil may slightly speed up a liver enzyme called CYP1A2. Bendamustine (Treanda, Belrapzo, Vivimusta) is partly broken down by that same enzyme. So in theory, adrafinil could clear your bendamustine a little faster, which might leave less of it around to do its job against the cancer.
I want to be honest about how soft this is. This is a theoretical concern based only on lab data, not real patient cases, and modafinil is only a weak booster of that enzyme. Still, bendamustine is serious chemotherapy, and you don't want anything nudging it the wrong way. Please loop in your oncology team or pharmacist before combining these.

What Is Bendamustine Injection Used For?
Brand names: Treanda, Belrapzo, Vivimusta
Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.
For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
