Grape and Valsartan

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Valsartan

Valsartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Grape interacts with.

It is unclear if grape juice or grape seed extract inhibits CYP2C9; research is conflicting.
In vitro evidence shows that grape seed extract or grape juice might inhibit CYP2C9 enzymes. However, a small pharmacokinetic study in healthy adults shows that drinking 8 ounces of grape juice once does not affect the clearance of flurbiprofen, a probe-drug for CYP2C9 metabolism. The effects of continued grape juice consumption are unclear.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: there's a theoretical concern that compounds in grape juice or grape seed extract might slow down a liver enzyme called CYP2C9, which plays a small role in clearing valsartan. If that enzyme were blocked, a bit more valsartan could linger in your system, which in theory might nudge its blood-pressure-lowering effect a little stronger (think lightheadedness, dizziness when standing, or feeling unusually tired).

But honestly, I wouldn't lose sleep over this one. The evidence is weak, coming mostly from lab dishes, and a small human study found grape juice didn't change clearance of a similar drug. Valsartan also isn't heavily dependent on this enzyme. Enjoying grapes or a glass of juice is fine. If you're considering concentrated grape seed extract supplements, just give your pharmacist or doctor a quick heads-up first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

Grape
  • Antioxidant support
  • Heart and circulation health
  • Vein problems (such as varicose veins)
  • Eye health
  • General anti-inflammatory support

Grapes and grape products like grape seed extract contain antioxidant compounds such as resveratrol and proanthocyanidins that may support heart and blood vessel health. About Valsartan

What Is Valsartan Used For?

Brand names: Diovan, Prexxartan

Valsartan is used to treat high blood pressure (hypertension) in adults and children, heart failure in adults, and to lower the risk of cardiovascular death in adults who have had a heart attack and have reduced heart function. Lowering blood pressure reduces the risk of stroke and heart attack.

Diovan (valsartan) tablets are approved to treat high blood pressure in children as young as 1 year old. Valsartan tablets (generic) are approved for children 6 years and older. Valsartan oral solution is specifically indicated for patients with heart failure or post-heart attack left ventricular dysfunction who are unable to swallow tablets.
