CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Sildenafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body clears sildenafil mostly through a liver enzyme called CYP3A4, and grape seed extract may nudge that enzyme one way or the other. The catch is that the science is genuinely mixed. Lab (test tube) studies hint it could slow that enzyme, which would let sildenafil build up and possibly bring on more headache, flushing, a stuffy nose, or a bigger drop in blood pressure. Animal studies suggest the opposite, that it could speed up clearance and make sildenafil work less well.
So far this hasn't actually been seen in people, so I wouldn't lose sleep over an occasional handful of grapes. Concentrated grape seed extract is the bigger question mark. If you take sildenafil regularly, especially for lung pressure, just run any grape supplement by your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
2 of 2Sildenafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
In vitro evidence shows that grape seed extract or grape juice might inhibit CYP2C9 enzymes. However, a small pharmacokinetic study in healthy adults shows that drinking 8 ounces of grape juice once does not affect the clearance of flurbiprofen, a probe-drug for CYP2C9 metabolism. The effects of continued grape juice consumption are unclear.

Here's the gist for sildenafil. Your body uses several liver enzymes to clear this drug, and one of them is called CYP2C9. The thinking is that compounds in grape (especially concentrated grape seed extract) might slow that enzyme a bit. If that happened, sildenafil could hang around a little longer, which in theory could nudge up its effects, things like headache, facial flushing, a stuffy nose, or lightheadedness from lower blood pressure.
But I want to keep this in perspective. The evidence here is weak, mostly from lab dishes, and a small human study found that drinking grape juice didn't change clearance of a similar drug. So for most people this is unlikely to cause trouble. Still, if you plan to take a daily grape supplement, it's worth a quick check with your pharmacist or doctor.

What Is Sildenafil Used For?
Brand name: Revatio
Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.
Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
