Here's the gist with St. John's wort and sildenafil. Your liver uses an enzyme called CYP3A4 to clear sildenafil out of your body. St. John's wort revs that enzyme up, so it can sweep sildenafil out faster than usual. The result is that you may end up with less sildenafil working in your system.

In practical terms, that can mean a weaker effect. For erectile dysfunction, the medicine may simply not work as well. For pulmonary arterial hypertension (Revatio), that's more concerning, since less blood vessel relaxation could let your symptoms creep back. This is a known, fairly likely interaction, especially with stronger St. John's wort products. Please don't change anything on your own. Loop in your pharmacist or doctor first.

Sildenafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

St. John's wort may increase the metabolism and reduce the levels of CYP2C9 substrates.
There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Here's the gist for sildenafil (Viagra or Revatio): St. John's wort tends to speed up some of the liver enzymes that break this drug down, including CYP2C9 and the bigger player CYP3A4. When your body clears sildenafil faster, less of it sticks around. So the practical worry is a weaker effect, a less reliable erection if you take it for ED, or less benefit for your lungs if you take Revatio for pulmonary hypertension.

Sildenafil is not a prodrug, so this isn't about a dangerous buildup, it's about the medicine possibly not working as well. The evidence here is mixed and only moderate, so it may not affect everyone the same way. Still, please check with your pharmacist or doctor before pairing these two rather than adjusting anything yourself.

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
