Grape and Drospirenone, Ethinyl Estradiol

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Grape interacts with whole categories of medication. Drospirenone, Ethinyl Estradiol falls into 2 of those categories, so it can interact with Grape in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Drospirenone, Ethinyl Estradiol

Drospirenone, Ethinyl Estradiol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.

Theoretically, grape juice might reduce the levels of CYP1A2 substrates.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the short version for your birth control pill (Yasmin, Gianvi, and similar). The estrogen part, ethinyl estradiol, is partly processed by a liver enzyme called CYP1A2. There's a small study suggesting grape juice might gently speed up that enzyme, which could in theory lower estrogen levels a bit and slightly weaken how well the pill works.

I want to be honest: this is mostly theoretical and based on weak evidence using a different drug, not your actual pill. A glass of grape juice or eating grapes is very unlikely to cause trouble. Concentrated grape seed extract supplements are a bigger unknown. Since reliable birth control matters, just run any grape supplement past your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Drospirenone, Ethinyl Estradiol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Your birth control pill (Yasmin, Gianvi, and similar) is partly processed by a liver enzyme called CYP3A4. The active hormones in this pill work as-is, so they're not a prodrug that needs switching on. That means if a grape seed extract slowed that enzyme down, hormone levels could creep up a bit (think nausea, breast tenderness, or spotting), and if it sped the enzyme up, levels could drop, which in theory could make the pill less reliable.

Here's the honest part: the lab and animal studies actually point in opposite directions, so nobody really knows which way it goes, and this has never been seen in real people. Eating grapes is not a concern. If you're considering a concentrated grape seed supplement, just run it by your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
