Here's the gist for your cough medicine. Dextromethorphan, the active ingredient in Vicks Formula 44, is partly cleared by a liver enzyme called CYP3A4. St. John's wort speeds that enzyme up, so your body may break down the dextromethorphan faster than usual. The likely result is simply that your cough syrup works less well, since less of it sticks around to quiet your cough.

There's a second wrinkle worth knowing. Both St. John's wort and dextromethorphan can nudge serotonin levels up, so combining them carries a small risk of too much serotonin (jitteriness, sweating, fast heartbeat, confusion). It's usually not dangerous for short-term cough use, but please run it by your pharmacist or doctor before pairing them.

Theoretically, St. John's wort might inhibit reuptake and increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Concomitant use of serotonergic drugs with St. John's wort can lead to increased adverse effects and increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstriction disorders.

Here's the thing about dextromethorphan, the cough suppressant in products like Vicks Formula 44: besides quieting a cough, it also nudges serotonin activity in the brain. St. John's wort does something similar. When you stack the two together, you can get too much serotonin, and that's where the concern lies.

In rare cases this can trigger what we call serotonin syndrome, with symptoms like agitation, a racing heart, sweating, shaking, or confusion. I'll be honest with you though, this warning is mostly theoretical and based on how these two work in the body, not on a pile of real-world reports. An occasional cough syrup dose is far less worrisome than heavy or long-term use of both. Still, it's worth a quick chat with your pharmacist before combining them.
