Why this applies to Lonafarnib

Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C9 substrates.
In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.

Here's the basic idea with Zokinvy (lonafarnib) and kava. Your body relies on a liver enzyme called CYP2C9 to help clear this medication. In lab studies, kava appears to slow that enzyme down. If that same effect happens in people, lonafarnib could build up higher than intended, which may mean more of its usual side effects like nausea, vomiting, diarrhea, loss of appetite, fatigue, or muscle and joint aches.

I want to be honest about the evidence, though. This is so far only seen in test-tube research, not in actual patients, so it's more of a theoretical caution. One thing that does stand out: both kava and lonafarnib can stress the liver, so combining them isn't something to do on your own. Please check with your pharmacist or doctor first.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on this pairing: the worry is mostly theoretical. Lonafarnib (Zokinvy) is cleared by a liver enzyme called CYP3A4, and early lab studies hinted that kava might slow that enzyme down. If that happened, lonafarnib could build up and you'd see more of its usual side effects, like nausea, vomiting, diarrhea, loss of appetite, or fatigue. But better-quality human research tells a calmer story. In one study, healthy people took a real kava dose for 28 days and it did not change CYP3A4 activity at all.

So the likely impact here is small. That said, both kava and lonafarnib can stress the liver, so I'd still loop in your pharmacist or doctor before combining them rather than deciding on your own.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
