Why this applies to Lonafarnib
Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Sesame interacts with.
In vitro, sesame inhibits CYP2C9. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: in test-tube studies, sesame seems to slow down an enzyme called CYP2C9, which is one of the pathways your body uses to clear Lonafarnib (Zokinvy). If that enzyme gets blocked, the drug could build up a bit higher than expected, which in theory might mean more of its usual side effects, things like nausea, vomiting, diarrhea, poor appetite, or feeling more tired.
The important part is the word theory. This has only been seen in lab and animal research, never reported in actual people, so it's far from proven. The sesame you sprinkle on food is very unlikely to matter. Concentrated sesame lignan supplements are the bigger question mark. Since Lonafarnib is a specialized medicine, it's worth a quick chat with your doctor or pharmacist before adding any sesame supplement.

Sesame is a nutritious seed and oil widely used in cooking and traditional medicine, and it provides healthy fats, fiber, and antioxidant compounds called lignans. What Is Lonafarnib Used For?
Brand name: Zokinvy
Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.
Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
