Here's the gist for Zokinvy (lonafarnib). The berberine in tree turmeric appears to slow down a liver enzyme called CYP2C9, which is one of the pathways that helps clear lonafarnib from your body. When that enzyme is partly blocked, the drug can build up and hit harder than intended. With lonafarnib that could mean more nausea, vomiting, diarrhea, loss of appetite, or fatigue.

I'll be honest, this is mostly a theoretical concern based on limited evidence, not strong human studies. But lonafarnib is a specialized, carefully dosed medicine, so I wouldn't add tree turmeric on your own. Please run it by your doctor or pharmacist first before combining the two.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs that are substrates of CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.

Here's the key thing about Zokinvy (lonafarnib): your body relies heavily on a liver enzyme called CYP3A4 to break it down and clear it out. The berberine in tree turmeric appears to slow that same enzyme. So if you take them together, your lonafarnib may not get cleared as quickly and could build up to higher levels than expected.

That matters because lonafarnib already tends to cause nausea, vomiting, diarrhea, poor appetite, and fatigue, and higher levels could make those worse. Lonafarnib is a carefully dosed medicine where blood levels really count, so even a modest change is worth taking seriously. This is mostly based on lab and early human data, not a flashy warning, but please check with your pharmacist or doctor before mixing them.

About Lonafarnib

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
